# Book a consultation

> Booking is a web form the patient completes themselves, because it needs their own details, consent and payment. A consultation is R350, payable before the appointment.

Complete the booking form at https://telehealthsol.com/book. There is no booking API.

## What to send before the appointment

Your full name, age, main concern or symptoms, current medication, any allergies, pre-existing conditions, surgical history, and your proof of payment.

## Fees

- Consultation: R350. A sick note consultation and a follow-up are the same R350, because each one is a consultation.
- Repeat prescription: R200
- Corporate consultation: R750
- Medical report: a further R500, charged after the consultation.

## Payment and confirmation

Consultation fees must be paid before the consultation. Send your proof of payment to confirm your booking.

## In an emergency

Do not use the booking form for chest pain, severe breathing difficulty, stroke symptoms, seizures, uncontrolled bleeding or loss of consciousness. Go to the nearest emergency department, or call 10177 (ambulance) or 112 from a mobile phone.
