## Confidential medical information

Everything you share during a consultation is treated as confidential medical information, handled securely and used only for your care.

Your records are held securely and seen only by the practice. Nothing goes further without your consent, unless the law requires it.

## What you share with the practice

To prepare for your consultation, the practice asks for your full name, age, main concern or symptoms, current medication, any allergies, pre-existing conditions and surgical history, along with your proof of payment.

Patients must provide accurate medical information, contact details, medication history and current symptoms to allow safe medical assessment and treatment recommendations.

## Consultations

Consultations are held one to one. Nothing is recorded, and no part of your history is discussed with an employer, insurer or family member without your explicit consent.

## Documents issued to you

Documents issued to you, including sick notes, prescriptions, referral letters and reports, are sent to you directly and kept as part of your confidential record with the practice.
