6. I consent to the collection, storage, and processing of my personal and medical information for purposes related to medical treatment, prescriptions, referrals, billing, and administration.
7. I understand that communication may occur via WhatsApp, telephone, video call, email, or other electronic platforms.
8. I understand that while reasonable security measures are used, electronic communication cannot guarantee complete confidentiality or security.
9. I understand that prescriptions and sick notes are issued at the doctor’s discretion and only where clinically appropriate.
10. Consultation fees are payable before the consultation and proof of payment must be provided prior to booking confirmation.
11. I understand that consultation fees are non-refundable once payment has been made and booking has been confirmed.
